Ibero’s Adult Health Home Care Management Services
Ibero’s Adult Health Homes provides Care Management services to ensure all those involved in an individual’s care are working together and sharing the necessary information in supporting a person’s recovery. A Health Home Care Manager is expected to help coordinate not just medical, mental health and substance abuse services, but the social service needs of the individual as well.
A Health Home Care Manager will assist you with:
Coordinating your care through effective communication with other providers
Advocating on your behalf so you receive the care you deserve
Navigating the complex healthcare system
Linking to resources for finding affordable housing, food, clothing, childcare and transportation
Learning more about meaningful activities to improve your health and keep you healthy
Identifying your goals and ensuring everyone involved in your care understands them and the care plan you created, to best help you reach those goals

Who is Eligible?
Medicaid recipients, including those who are already members of a Managed Care Organization. Individuals who have both Medicaid and Medicare are also eligible. Those eligible also need to meet one of the following conditions:
Two or more chronic health conditions, such as asthma, diabetes, heart disease, mental health condition or substance use disorder,
or a significant Mental Illness,
or living with HIV/AIDs.
